No, just fill in the top section. Leave the `by' section alone, as you
ARE your own representative.

The only time you'd fill out the bottom section is if you were buying
a license for a company, e.g

	Sproggs Inc.
	5 Looney road,
	SPOTSWOLD. NSW. 2001

	by

	Warren Toomey
	etc etc etc.
