Are the classes for <NOTE> defined in the spec? Or are they just examples
of classes of <NOTE>s one might want to use? If the former, this seems to
be an inconsistency in the spec. If the latter, perhaps the doc should be
updated to make it clear that the classes are suggestions, not
"hard-wired".
