Changing servers

How do you go about changing servers without changing the home page
URL address?

I assume this can be done through DNS, but I don't know how (I am an
inexperienced system admin).

For example, say you have X.URL as your URL which gets resolved to
X.IPaddress. We are changing to a different server in a different domain,
so I want X.URL to get resolved to Y.IPaddress.

I am not sure who I notify via DNS to make sure the old DNS record is removed, and
that my new DNS record gets propagated to all the right places.

Any help would be appreciated.
