HELP: Examples of FORMs and INDEXing on W3/HTML

I have browse thru 5 hours of WEB info space, but I have nto been able to
find any test servers, or exmaples out there on FORMS and INDEX Servers.

Would someone be so kind to send or direct an FTP list files that I can
get some good examples on these topics?

Examples of FORMS to interact with EMAIL will especially be helpful.

Examples of setting up an environment that support Index Search on
topic would be great.
