Steve Putz would rather have changed marked by EM pairs, and would
be willing to split up a change that crossed PARA boundaries
into two changes. If you could retain the same ID for the two
resulting EM start tags, that might be just barely acceptable.
But you can't; it's an error according to SGML. So you'll
have no way to know that the change that goes to the end of
PARA foo is the same change that begins the next PARA, bar.

So we need to use EMPTY elements instead of elements with
content.
